For Parents

Below are some helpful checklist items and links to required forms. 

  • All players must have shin guards for games and practices 

  • Uniform orders and fees are due Friday August 16th. 

  • Club fees are due Friday August 23rd. Fees include player registration, field usage/maintenance, and referee costs. 

  • Complete your players profile on GotSport including player photo that will be used to print player cards.

  • Complete the International Transfer Clearance (ITC) Questionnaire under requirements. 

  • Upload the following completed documents:

    • Medical Release form ​

    • CDC Concussion Awareness 

    • Code of Conduct

    • Proof of player age 

      • Birth Certificate

      • Passport

      • Adoption Certificate

      • Certificate of Naturalization 

      • School Record

  • Contact your players coach for help navigating GotSport. The following instructions may also be helpful.​​​​​
